Notice of Retirement and Closing of Practice

Dear Patients, Friends, and Community,
After 42 years of practicing medicine, I have decided to retire. My final day seeing patients at The Institute for Lifestyle, Preventive, and Family Practice will be September 30, 2026. It has been a true privilege to care for you and be part of this community’s health and lives over the years.

Accessing Your Medical Records:

Your medical records are confidential. You have the right to obtain a copy of your records or request that they be transferred to another physician.
● Prior to September 30, 2026: You may complete a Medical Records Release Form which is available at the office.

● After September 30, 2026: Your medical records will be securely kept by Dr. J. Koch MD located at 1224 S. John Redditt Dr. Lufkin, TX (936) 639-4394. We wish you the very best in your future health and well-being.
